Financial Aid Applications and Forms
UA Cossatot’s Financial Aid Office is pleased to make financial aid forms, verification requests, and tuition waiver applications accessible to students online. To download one of the forms listed below, simply click on the hyperlink of the form you need to pull up a downloadable PDF file. From there, you may print your desired form, and submit it to any UAC Financial Aid Office by mail or fax. Forms submitted to the Financial Aid Office must be legible with all required signature fields filled out.

If your FAFSA is selected for verification, the Financial Aid Office will notify you as soon as possible. You may be required to submit one or more of the additional documents listed below.

The Free Application for Federal Student Aid (FAFSA) form does not immediately provide
a place for students and/or their families to explain special financial circumstances.
The term "Professional Judgment Review" refers to the school’s authority to adjust the data elements on the FAFSA to override
a student's status in some circumstances. The decision of the financial aid administrator
is final. There is no appeal after a Professional Judgment Review.
Examples of FAFSA "Special Circumstances" are as follows:
- Unusual medical and dental expenses (expenses that exceed 7.5% of adjusted gross income)
- Support of extended family
- Income reduction or non-recurring income
- Divorce or death of a parent
- Dependency status

FAFSA Dependency Override:
Many students feel they are independent because they currently live on their own or because their parents no longer claim them on their income taxes. Others feel they should be considered independent because their parents refuse to provide information on the FAFSA or because their parents cannot afford to help with college expenses. However, these reasons are not sufficient for a financial aid appeal.
The UAC Financial Aid Office is required to consider parent information and expect a parental contribution for students who are not independent according to the FAFSA definition unless exceptions are made. Exceptions are made only when adequate documentation of extenuating circumstances exists. Extenuating circumstances are generally defined by a student’s inability to have contact with their parents. Review the reasons for appeal in the document links below and check the one that best describes your circumstances. If none of these circumstances apply to your situation, do not complete this form.
